Klippel-Feil Syndrome

What it is

Klippel-Feil syndrome is a condition present from birth in which two or more of the neck vertebrae are fused together — they formed joined rather than separate during early development. It is the most common congenital fusion of the cervical spine. Classic features can include a shorter neck, a low hairline at the back, and limited neck movement, though many people have only mild findings.

What it feels like

Many people live well with it and have few symptoms. Because the fused segments do not move, the healthy levels next to them take on extra work and can wear faster over the years, which may bring neck pain or stiffness in adulthood. It is also sometimes associated with other findings elsewhere in the body, so a broader check is worthwhile at diagnosis.

How we diagnose and monitor it

X-rays show the fused vertebrae, and a CT or MRI gives more detail about the bones, the canal, and the spinal cord. Because the levels next to a congenital fusion bear extra stress, I keep an eye on them over time, and I watch for any instability or narrowing that could affect the cord.

How we treat it

Most people need no surgery — care centers on monitoring, physical therapy for neck comfort, and sensible activity guidance, since a stiff, congenitally fused neck has less ability to absorb a hard impact. Surgery is reserved for specific problems that develop, such as instability or compression of the cord at an adjacent level. The aim is to protect the healthy segments and the cord.

The science — for the curious

The central long-term issue in Klippel-Feil is the load placed on the levels next to the congenital fusion: with motion concentrated into fewer segments, those neighbors can become hypermobile or wear more quickly — the same adjacent-level stress we try to limit whenever we fuse a spine surgically, here present from birth. That is why monitoring those adjacent levels, and protecting the neck from injury, are the heart of care.
